    If you’re dealing with both constipation and stubborn breakouts, you might be wondering if the two are connected. It’s a fair question, especially given all the buzz around gut health on social media these days.

    The short answer: Constipation doesn’t directly cause acne, but the two conditions may share some common underlying triggers. Here’s what dermatologists want you to know about the gut-skin connection and how to tackle both issues.

    Does Constipation Lead to Breakouts?

    Most people have dealt with constipation at some point—but let’s start with what constipation actually means. While definitions vary, most health professionals consider fewer than three bowel movements per week—or passing hard, dry stools—to be constipation. The cause? Not drinking enough water, eating too little fiber, skipping physical activity and ignoring the urge to “go” can all contribute. Stress and changes in routine—think travel or a new schedule—are also common culprits. For some, underlying health conditions such as irritable bowel syndrome or thyroid disorders may play a role.

    With growing interest in gut health and its integral role in chronic diseases, immunity, and more, the connection between our microbiome and skin health has also become an active area of research. Dermatologists acknowledge that constipation and acne often show up together, but here’s where it gets nuanced. While you might wonder if a backed-up gut would show up on your skin, the research doesn’t support a direct cause-and-effect relationship between constipation and acne.

    “Constipation doesn’t directly cause acne, but the two can be linked,” explains Dr. Geeta Yadav, a board-certified dermatologist. “Slower gut transit can be associated with changes in gut bacteria and low-grade inflammation, which may contribute to acne. Often, constipation and breakouts share the same underlying drivers, such as stress, dehydration, low fiber intake, poor sleep or diets high in refined and ultra-processed foods.”

    Board certified dermatologist Dr. Caren Campbell, M.D., FAAD agrees, “There is no good evidence at this time to support inquiring about bowel habits with acne.” Instead, Campbell focuses on other factors when treating patients with breakouts. “There is good evidence your gut microbiome can worsen acne, so I prefer to focus on dietary improvements or changes rather than focusing on bowel habits, which may or may not be impacted by changes in the microbiome.”

    Understanding the Gut-Skin Axis

    You may have heard the term “gut-skin axis” floating around wellness circles, and there’s legitimate science behind it. This concept describes the two-way communication between your digestive system and your skin.

    “The gut-skin axis is a way of describing the communication between the gut and the skin,” says Yadav. “The gut microbiome and the gut lining help regulate immune responses and inflammation throughout the body. When that system is out of balance, inflammatory signals can increase, and in some people, that may worsen inflammatory skin conditions like acne.”

    However, Yadav notes that this is just one piece of a much larger puzzle. Acne is influenced by many factors, including genetics, hormones, skincare habits and environmental exposures. Gut health matters, but it’s not the whole story.

    How to Manage Constipation and Support Clear Skin

    If you’re dealing with both constipation and breakouts, the good news is that many of the same lifestyle changes can help address both concerns. “If someone is dealing with both constipation and breakouts, I usually recommend addressing both at the same time rather than focusing on just one,” says Yadav.

    Here are dermatologist-backed strategies to consider:

    • Stay hydrated. Drinking enough water helps keep stool soft and easier to pass. Proper hydration also supports healthy skin by maintaining moisture and helping flush out waste products.
    • Gradually increase fiber intake. Fiber adds bulk to stool and promotes regular bowel movements. Aim for a variety of fiber-rich foods like fruits, vegetables, whole grains, legumes, nuts and seeds. Increasing fiber slowly helps prevent bloating and gas.
    • Move your body regularly. Physical activity stimulates the muscles in your intestines, helping move things along.
    • Build a consistent bathroom routine. Try to go at the same time each day, ideally after a meal when your digestive system is naturally more active. Don’t ignore the urge when it strikes.
    • Focus on whole, minimally processed foods. “Emphasizing whole, minimally processed foods, plenty of plants and adequate protein helps support regular bowel habits and stable blood sugar,” says Yadav. “Increasing omega-3 intake may help reduce inflammation.”
    • Don’t abandon your skincare routine. While you work on gut health, keep up with proven acne treatments. Yadav recommends continuing gentle skincare and evidence-based acne medications, escalating to prescription treatments if needed.
    • Manage stress and prioritize sleep. Both stress and poor sleep can disrupt digestion and trigger breakouts. Finding healthy ways to manage stress—whether through exercise, meditation or time with loved ones—benefits your gut and your skin.

    When to See a Doctor

    If lifestyle changes aren’t providing relief, it may be time to seek professional guidance. “If you are struggling with constipation, I would encourage you to see your pediatrician or primary care doctor, with possible referral to a gastroenterologist if appropriate, to discuss ways to address the constipation and ensure something more serious is not being missed,” advises Campbell.

    As for persistent breakouts? “If you are struggling with acne, I would encourage you to see a dermatologist to treat your acne to prevent permanent scarring and get the most evidence-based, timely care for you and your skin,” she adds.
