    All About Vitamin K2 and Why Quality Matters

    Here’s the paradox: people consume calcium with the intention to build strong bones, but some of that calcium may end up in the arteries and soft tissues instead of bones.[1] One way to help direct calcium where it’s needed? Vitamin K2.

    Vitamin K exists in two natural forms with distinct roles and behaviors. Vitamin K1 (phylloquinone) dominates dietary intake through green leafy vegetables, while vitamin K2 (menaquinones) appears in animal foods, certain cheeses, and fermented products. The structural difference — Vitamin K1 contains a phytyl side chain composed of four partly saturated isoprenoid units, whereas vitamin K2 (menaquinones) have side chains with a variable number of repeating, unsaturated isoprenoid units (MK‑n), leading to distinct biological behavior. [2]

    K1’s half-life spans just 1-2 hours. After absorption, it concentrates in the liver where it activates blood clotting factors before rapid clearance.[3] K2 (specifically MK-7) has a half-life of three days, allowing continuous delivery to extrahepatic tissues including bones and arteries.[3]

    Tissue distribution follows these pharmacokinetic patterns. Studies show K1 rapidly accumulates in liver tissue while remaining scarce in bone and vessel walls. MK-7 achieves sustained serum levels that support protein carboxylation throughout the body.[3]

    The Menaquinone Family

    Menaquinones span from MK-4 through MK-14, each defined by the number of unsaturated isoprenoid units. Not all menaquinones perform equally.

    MK-4 appears in animal products (eggs, meat, dairy) but demonstrates poor bioavailability at nutritional doses. When researchers gave healthy women 420mcg of MK-4, serum levels remained undetectable at all time points. The same dose of MK-7 produced measurable serum concentrations for 48 hours.[4] Seven consecutive days of 60mcg MK-4 supplementation failed to increase serum MK-4 levels. MK-7 supplementation at the same dose raised serum levels significantly in all subjects.[4]

    MK-7’s longer side chain creates the optimal balance: sufficient lipophilicity for absorption paired with extended circulation time. This explains why MK-7 dominates the supplement market and clinical research.[5]

    Western diets provide minimal K2, though small amounts are found in eggs, cheese and meat.

    The K2 Quality Crisis

    Understanding which form of vitamin K2 to take matters little if the product fails to deliver what the label promises. When Balchem partnered with a third-party laboratory to test 38 K2 supplements for MK-7 content and isomeric purity, the results exposed a troubling truth: 71% failed to meet both quality standards.

    What Consumers Don’t Know

    The problem centers on isomeric purity. Only the all-trans configuration matches the shape found in nature. Cis isomers don’t activate vitamin K-dependent proteins effectively. They occupy space on the label and in the capsule without delivering proportional benefits. While all-trans isomers fit the keyhole to unlock the activation of K-dependent proteins, cis-isomers have a different shape and are less active.[6]

    The United States Pharmacopeia (USP) established 98% all-trans as the minimum acceptable standard. Many products fall short.

    The analytics challenge explains how substandard products reach consumers. Standard testing methods measure total vitamin K2 content with the sum of all isomers present. A product can legitimately claim “25mcg vitamin K2 per serving” while containing mostly inactive cis forms. Balchem’s investigation revealed products with 25mcg total K2 but only 4mcg of bioactive all-trans MK-7. That’s 84% inactive material.

    Unfortunately, USP monograph-compliant methods that specifically measure isomeric purity remain uncommon in routine testing. Without this analysis, manufacturers and consumers alike remain unaware of what’s actually in the bottle.

    Fortunately, these are solvable problems:

    K2VITAL: Setting the Standard

      Vitamin K2 shows greater activity in the carboxylation of Vitamin K dependent proteins such as osteocalcin and MGP compared to vitamin K1.[8]

    • Osteocalcin: Directing Calcium Into Bone

      Osteoblasts (bone-building cells) produce osteocalcin during bone formation. When carboxylated by vitamin K2, osteocalcin binds calcium ions and incorporates them into bone mineral matrix — the structural foundation that gives bones their strength and density.

      A single 420mcg oral dose of MK-7 (open circles) produced measurable serum vitamin K2 levels for up to 48 hours, while MK-4 (filled squares) remained undetectable at every time point.[4]

      Undercarboxylated osteocalcin (ucOC) cannot bind calcium effectively. Clinical trials measure the ucOC-to-total-osteocalcin ratio as a functional marker of K status. Supplementation with MK-7 consistently reduces ucOC levels while increasing the carboxylated fraction, demonstrating improved protein activation.[5]

      The three-year Knapen bone health trial (detailed in the Health Benefits section below) confirmed this mechanism in practice: 180mcg daily MK-7 supplementation increased the carboxylated fraction of osteocalcin while improving bone strength indices compared to placebo.[9]

    • Matrix Gla-Protein: Supporting arterial health

      Vascular smooth muscle cells produce MGP to prevent calcium deposition in arterial walls. When properly carboxylated by K2, MGP binds calcium crystals and maintains vessel wall health.

The Knapen 2015 trial demonstrated that three-year MK-7 supplementation reduced circulating dp-ucMGP by 50% while decreasing arterial stiffness in postmenopausal women with elevated baseline values.[10]

      Adequate K2 intake activates both proteins simultaneously, directing calcium toward bone while supporting healthy levels of calcification — solving the calcium paradox through targeted protein activation.

    With that explained, we can get back to the real-world benefits:

    Health Benefits of K2 MK-7

    While vitamin K2 is best known for its role in bone and cardiovascular health, its functions extend beyond these two areas. The following sections focus specifically on these well‑established benefits:

    • Bone Health

      The three-year intervention trial by Knapen and colleagues stands as the definitive demonstration of MK-7’s bone health benefits. 244 healthy postmenopausal women received either 180mcg daily MK-7 or placebo. After three years, the MK-7 group showed more bone mineral density at both the lumbar spine and femoral neck. More importantly, bone strength improved in the MK-7 group but not in placebo.[9]

      This epidemiological finding gains mechanistic support from intervention trials. The Knapen 2015 cardiovascular study measured arterial stiffness and vascular biomarkers in postmenopausal women supplementing with 180mcg daily MK-7 for three years. Women with high baseline arterial stiffness showed significant improvement, with decreased carotid-femoral pulse wave velocity and improved arterial elasticity. Circulating dp-ucMGP levels decreased by 50% relative to placebo.[10]

    Dosing and Formulation Guidance

    Effective Dosage Range

    Clinical trials establish a dose-response relationship for MK-7 supplementation. The Møller 2016 study tested three doses (45mcg, 90mcg, and 180mcg daily for six weeks), measuring both serum MK-7 concentrations and osteocalcin carboxylation. All three doses significantly increased carboxylated osteocalcin and reduced undercarboxylated osteocalcin compared to placebo, confirming biological activity across the entire range.[7]

    The dose-response was non-linear: 45mcg produced substantial effects, while 90mcg and 180mcg generated progressively greater serum concentrations without proportionally greater carboxylation improvements.

    Long-term trials support this range of 90-180 mcg for higher serum concentrations. The Knapen study used 180mcg daily for three years with excellent safety profiles and documented benefits for bone and cardiovascular health.[9,10]

    Theuwissen 2014 demonstrated that even lower doses improve vitamin K status markers, though higher doses may be necessary for maximal protein carboxylation in individuals with baseline insufficiency.[11]

    As a fat-soluble vitamin, K2 absorption improves when taken with meals containing dietary fat. The Møller 2016 bioavailability study administered supplements in conjunction with a standardized breakfast containing approximately 25g fat to ensure optimal uptake.[7]

    Formulation Considerations
